and stroke. The goal of the program is to offer you information,
tools and support so you can make lifestyle changes that will
cut your risk. This program can help you identify your risk
factors and then give you tailored information to help lower
them. Even if your risk is low, it can help you stay healthy.
What kind of information will I receive?
All the information you get
will be tailored just for you — from health information to
tips on staying physically active. We draw upon 50 years of
research to give you the right information. Imagine getting
information customized just for you on these topics:

- Physical Inactivity
- Nutrition
- Medication Compliance
You'll get not only facts and figures, but
information on how to start following a healthier diet, how
to increase the physical activity you get and more. And it's
all customized to your own needs! We want to make it as easy
as possible for you to make good lifestyle changes that you
can maintain for a lifetime.
How is the program personalized to me? First,
take the time to carefully complete your personal Health and
Behavioral Assessment. When you've done that, a unique program
based on the information you provide is created. With the
accurate information that you give us about your health and
lifestyle, family history and personal preferences, information
is tailored to your unique profile. Whenever you update your
profile, the information is changed and sent to you to reflect
those changes.
The program will help you get over the hard
parts of making lifestyle changes — such as quitting smoking,
following a healthier diet, adding physical activity to your
normal routine, and taking your medications as prescribed.
You choose to make the changes, and the AFA provides support
to assist you.
